Peter Bretter, an accomplished composer, has been in a committed relationship with acclaimed actress Sarah Marshall for five years. One day, as Peter stands naked in their apartment, Sarah tells him she's breaking up with him. In an effort to cope with his emotional anguish, Peter escapes to Turtle Bay Resort in Hawaii. However, upon arrival, he discovers that Sarah and her new beau Aldous Snow (Russell Brand), who is coincidentally a British rock star, are also staying at the same location. Rachel Jansen (Mila Kunis), a gracious hotel concierge, generously provides Peter with an extraordinarily expensive suite at no cost in exchange for him tidying the room up himself.

After Sarah breaks up with Peter , he has several nights of heartache with Sarah’s TV show and a big bowl of cereal. Peter’s step brother Brian (Bill Hader) can’t see Peter in such condition, so he takes Peter to the bar. In the bar Brian tries to convince Peter that he needs some rest at the resort to have some time for himself, his art, to write some new music. But Peter only wants to hook up with somebody. They agree to sit in the bar and talk to some ladies for a while, and Peter will go on vacation.

Sitting on the beach, Peter and Rachel have a nice conversation. But suddenly Rachel’s ex appears on the beach. Rachel tells Peter that he should run if she runs. Rachel starts to argue with her ex, telling him that he shouldn’t be here. Peter decides to be a gentleman, so he decides to talk peacefully with the guy, but they soon begin to fight.

Peter and Rachel then run away to the bar. Having a beer together, Peter tries to continue to be a gentleman, but Rachel tells him that she doesn’t need it, she can help herself, and that he should get more beer for them. Peter remembers how he always was the carrier of Sarah’s bags and stood away from her during red carpet events.

Rachel suggests that they jump, but a shocked Peter wants to kiss Rachel. Instead she jumps off the cliff, so he has nothing to do but follow.
